Montbello High School has been selected by TeamSmile and the Colorado Rockies, for our students to receive FREE dental care!! This service will be for MHS students only, it will be held at Coors Field on Friday, May 3rd. MHS will provide transportation. See the above flier for more information.
In order to participate and receive free dental care, a parent or guardian must fill out the consent form below. Please fill out either the English or Spanish form and return it to our MHS Nurse, Ms. Amanda Webster. All forms can be found on our website here, or physical copies are available in the Main Office.
